As a member of the Health Support Center (HSC) team, you'll support those that are in our facilities who are interfacing and providing care to our patients and community members to positively impact our mission of making communities healthier .More about our teamJoin our team in a 90-bed Skilled Nursing & Rehabilitation Unit, with 20 Rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and Medication Aides. Operating with a patient ratio of 1:15 days, 1:19 evenings and 1:25 nights, our unit cares for patients with Congestive Heart Failure (CHF), COPD, Diabetes, ESRD, Renal Failure, Dementia, Stroke, and Heart Disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.How you'll contributeA Registered Nurse (RN), who excels in this role:Accurately performs patient assessments and identifies patient needsIdentifies and initiates appropriate nursing interventionsProvides care appropriate to condition and age of the patientPerforms timely and appropriate documentation relating to medical necessity in the medical recordResponsible for completion and revision of the Interdisciplinary Care Plan for each patientPerforms timely and accurate QI assessmentsWhy join usWe believe that investing in our employees is the first step to providing excellent patient care.
