LutherPark Community is seeking an RN/LPN for Trinity Center at Luther Park Community,a 120-bed skilled nursing facility which specializes in Alzheimer's care. Ifyou are a skilled, compassionate nurse that is dedicated to making sure thebest quality of care is given, we want to talk to you!JOBSUMMARY:TheCharge Nurse will be responsible for planning, organizing and carrying outnursing care of our residents. Responsibilities include orientating,instructing and supervising our CNAs, CMAs and resident care specialists. TheCharge Nurse complies with established policies and procedures and performsprofessional nursing duties within scope of licensure.RESPONSIBILITIES:• Gives andreceives report. Updates assignment sheets for CNAs, CMAs and resident carespecialists• Carries out orassigns nursing duties to other personnel• Contacts physiciansas needed for change in condition of resident• Informs residentand families of new physician order(s) and changes in Plan of Care withinmandated time frame• Keeps DON, ADONand evening shift supervisor informed of resident status and other pertinentmatters• Admits andorients new resident and involves family with orientation• Checks physician'sorders, transfer forms and pre-admission information; reviews resident'smedical and social history; completes admissions and documentation forms; ensuresresidents supplies and medications are ordered and equipment is working properly• Maintains properdocumentation of facility records, i.e., narcotic records, medical chart forms,weekly skin assessment and update wound pictures weekly• Maintains daily,weekly or monthly weight and vital signs and informs DON, ADON and On CallManager of any changes• Dischargesresidents to the community or other facilities. Contacts family prior to anytransfers to hospital and ensures resident is escorted to transportation ortransfer vehicle, completes required forms/documentation• Performs,supervises or assists with resident's care according to policies andprocedures. Assigns CNA, CMA or resident care specialist to daily routines andcheck all staff charting is complete every shift• Prepares andadministers medications and treatments as prescribed.
