MindDivers is hiring a Psychiatric Nurse Practitioner (PMHNP) to lead outpatient psychiatric evaluation, diagnosis, and medication management for adolescents and adults. This role owns the full med-management lifecycle—assessment through follow-up—while partnering tightly with therapy, care management, and billing/compliance to drive excellent outcomes and clean claims. Key Responsibilities Assessment & Diagnosis Comprehensive psychiatric evaluations and focused follow-ups Differential diagnosis; biopsychosocial formulation Use of validated scales (e.g., PHQ-9/GAD-7) as clinically appropriate Medication reconciliation and ongoing monitoring Treatment Planning & Prescribing Evidence-based pharmacotherapy with informed consent and patient education Lab/ECG monitoring when indicated; side-effect mitigation and deprescribing PDMP checks (CURES) and safe controlled-substance workflows Risk Management Structured suicide/self-harm/violence assessments; safety plans Mandated reporting; crisis triage; 5150/5585 interface per policy Care Integration Coordinate with therapy, primary care, SUD providers, case management, and community resources Referrals to higher levels of care; consult Medical Director on complex/high-risk cases Therapeutic Interventions (as scope allows) Brief psychotherapy/MI, psychoeducation, adherence counseling, relapse prevention Clinical Documentation & Coding Real-time EHR documentation; accurate problem/med/allergy lists ICD-10, CPT/E/M accuracy; time-based documentation for prolonged services when applicable Meet payer, legal, and internal audit standards; correct errors per policy Quality, Compliance & Equity HIPAA, OSHA/IIPP, DEA, and CURES compliance Culturally humble, LGBTQ+-affirming, recovery-oriented care Panel & Schedule Management Template hygiene, recall lists, refill protocols, lab follow-ups, inbox governance
