Care Co-Ordinator
Operational Oversight: Support the smooth running of our care delivery from the office, ensuring staffing levels, rota management and care delivery meet assessed needs.
Quality & Compliance: Monitor care diary logs, documentation, care plans and daily records to ensure compliance with CQC and safeguarding requirements.
Staff Support: Supervise and support Team Leaders and Personal Assistants promoting professional development and best practice.
Care Planning: Review and update person-centred care plans, risk assessments and all relevant documentation regularly.
Liaison: Act as a key point of contact for families, professionals and outside organisations to maintain strong working relationships and collaborative care.
Incident Management: Oversee incident reporting as they are received into the office and follow-up actions as required
Recruitment & Induction: Participate in the recruitment and onboarding of new staff members where required.
Administrative Duties: Maintain accurate records, handle correspondence and ensure effective communication across the organisation.
On Call: Provide an on call support service on a shared rota basis ensuring continuity of service and effective out of hours management

This role will be based at our Head office in Hartford, Cheshire 3 days per week and home working 2 days per week however you may also be required to commute to client homes across the Northwest at times of service need.

Person Specification Care Support Worker Driving Licence & use of your own vehicle for work purposes
Minimum of 2 years experience in Health and Social care.
NVQ level 3 or willing to work towards.
