This is a meaningful opportunity to make a direct impact on the lives of vulnerable adults while working in a supportive and mission-driven environment.About the RoleAs a Care Coordinator, you will serve as a trusted advocate and central point of connection for clients, families, providers, and community resources. You will spend approximately half your week conducting in-person client or care team meetings and the remainder coordinating services, documentation, and follow-up.This position supports individuals who are chronically ill or living with neurological or mental health diagnoses. Many clients reside in long-term care settings. A full-time caseload ranges from 25–50 clientsYour work will help ensure each client's safety, dignity, and greatest degree of independence within the least restrictive environmentKey ResponsibilitiesPromote timely access to appropriate medical and supportive careDevelop, implement, and monitor individualized care plansCoordinate communication between clients, families, hospitals, clinics, specialty providers, and community partnersManage transitions of care and referralsSupport personal care needs (groceries, personal care items, durable medical goods)Maintain complete and accurate electronic recordsIdentify at-risk clients and contribute to program improvement effortsReassess client status and adjust care plans as needs evolveQualifications3–5 years of experience in clinical or community-based care settings (reduction considered for licensed professionals)Strong organizational skills and ability to maintain accurate documentationComfort using communication technologies and electronic systemsKnowledge of local healthcare and community resources is a plusValid driver's license and reliable personal vehicle requiredSchedule & Work EnvironmentFull-time: Monday–Friday, 8:00 a.m. – 5:00 p.m. (Part-time may be considered)Travel throughout Vermont and New Hampshire requiredRemote work available when not travelingCompany cell phone providedAthena offers a collaborative and supportive culture, paid time off, and opportunities for professional growthWhy Athena?At Athena Advocacy, you will do more than coordinate care—you will be a steady, trusted presence for individuals, navigating complex health and life transitions.
