RN-Case Manager
Registered Nurse (RN) – Case Manager
Location: Burbank, CA
Assignment Duration: 13 weeks
Schedule: Day Shift | 8‑hour shifts, 5 days per week (40 hours weekly)
Start Date: August 31, 2026
Compensation: $2,140 – $2,900 weekly gross
Responsibilities
Perform comprehensive care coordination and case management for acute medical patients.
Collaborate with physicians, nurses, and interdisciplinary teams to develop individualized care plans.
Facilitate discharge planning, ensuring continuity of care and appropriate follow‑up services.
Document patient interactions and care plans accurately in Epic EMR.
Advocate for patients and families to optimize resource utilization and improve outcomes.
Requirements
Active RN license in California
Minimum 2 years of acute medical Care Manager experience (care coordination/discharge planning)
Proficiency with Epic EMR
