This role focuses on delivering patient-centered hospice care, coordinating services, and ensuring continuity of care through an interdisciplinary approach.✅ Must-Have Skills:✔ Active Montana RN License✔ Strong hospice, home health, or acute care experience✔ Excellent patient assessment and care coordination skills✔ Ability to manage caseload and work independently✔ Strong communication and interpersonal skills Experience Required:✔ Minimum 3 years of RN experience✔ Hospice or home health experience preferred✔ Experience with clinical skills such as wound care, IV therapy, catheterization, etc. Key Responsibilities:• Provide skilled nursing care in patient homes• Perform assessments, care planning, and implementation of treatment plans• Coordinate care with interdisciplinary teams• Educate patients and families on disease management and care plans• Monitor patient conditions and adjust care as needed• Maintain accurate documentation in EMR (Homecare Homebase preferred)• Participate in on-call rotation and respond to patient needs• Ensure compliance with care standards and regulatory requirementsCloudinfraIT Inc Job ID #296152.
