Apply nursing skills where the patient calls home with oversight of hospice healthcare needs.Independent work with the support of an experienced team. Ability to practice at the top of your license. Identify patient needs with the help of an interdisciplinary team. Provide patient assessments. Design and implement care plans. Assist with setting and achieving patient health goals. Connect patients with resources.
