We encourage you to apply today!Responsibilities:Provide patient care according to client's care plan and needsPerform wound care treatments and assist clients with medication managementTeach the client and/or family members about care and support skillsCommunicate regularly with RN Case Managers, Supervisors, and the Care TeamQualifications:Licensed as a Practical Nurse / LPN in the state of MinnesotaDriving is required; must have a reliable vehicle with current insurance and a valid Driver's LicenseDemonstrated ability to work autonomouslyPrior Nursing experience in a TCU, Hospice, or Home Health settingExperience with Electronic Medical Records (EMR) software is preferredMilitary Occupational Classification (MOC) Codes: If you're a military member transitioning to civilian life, we value your service and skills.
