Referral Coordinator
Location: New Horizons HealthcareDepartment: NursingReports To: Director of NursingEmployment Status: Full-TimePay Rate: $20.15/hr.Position Summary:The Referral Coordinator plays a critical role in ensuring seamless coordination of specialty care, diagnostic services, and follow-up appointments for patients. This position is responsible for managing referral orders, communicating with external specialists, verifying insurance requirements, and supporting patients in accessing needed services. The Referral Coordinator helps ensure continuity of care and supports compliance with HRSA, PCMH, and FQHC quality standards. This position follows a Monday through Friday schedule and is hybrid, with the ability to work remotely.Key Responsibilities:Process incoming provider referrals promptly and accurately.Coordinate appointments with specialists, diagnostic imaging centers, and ancillary providers.Obtain prior authorizations and verify insurance coverage as needed.Track and follow up on referral outcomes to ensure reports are received and uploaded into the EHR.Contact patients to provide referral information and instructions.Assist patients with scheduling and address any barriers to accessing referred care.Accurately document all referral activities and patient communications in eClinicalWorks (eCW).Ensure referrals are processed according to HRSA, UDS, and PCMH guidelines.Collaborate with providers, care coordinators, and front desk staff to optimize referral workflows.Assist with audits and reporting for UDS and HRSA quality measures.Qualifications:Experience in a healthcare setting managing referrals, scheduling, or care coordination.Knowledge of insurance verification and prior authorization processes.Proficiency with Electronic Health Records (eClinicalWorks preferred).Strong communication and organizational skills.Preferred:Experience working in an FQHC or outpatient primary care clinic.Familiarity with HRSA and UDS reporting standards.Key Skills and Attributes:Attention to detail and accuracy.Ability to prioritize tasks and manage multiple referrals simultaneously.Strong problem-solving and patient advocacy skills.Commitment to providing equitable, patient-centered care.OCCUPATIONAL HEALTH AND SAFETY Approximately 75% of work may be performed in a typical office setting, with the remaining time eligible for remote work depending on operational needs.
