Family Practice/Primary Care Nurse Practitioner

Overview An outpatient primary care organization is seeking a Spanish-speaking Nurse Practitioner to join a value-based clinic in the Bronx, New York .This clinic focuses on providing preventative and chronic care management for Medicare patients through a collaborative team model and smaller patient panels.The care model prioritizes patient outcomes, preventative medicine, and stronger provider-patient relationships. Position Highlights • Schedule: Monday Friday• Hours: 8:00 AM 5:00 PM• Setting: Outpatient primary care clinic• Patient population: Primarily geriatric / Medicare• Visit length: 30 minutes• Patient panel capped around 450• EMR: EpicClinical Support Each provider receives:• Dedicated medical assistant• Dedicated scribe• Shared RN support• Collaborative physician and APP care teamsThis structure allows providers to focus on quality patient care rather than high-volume visits. Responsibilities • Provide comprehensive primary care services• Diagnose and treat acute and chronic conditions• Conduct preventative health visits• Educate patients on disease management and lifestyle improvements• Document encounters in Epic EMRRequirements • Family Nurse Practitioner (FNP) or Adult-Gerontology NP• Board Certified• Active New York NP license • Active DEA license• Fluent or professional Spanish proficiency required Experience with Medicare or geriatric patient populations preferred.New graduates may be considered.Compensation & Benefits Compensation is competitive and based on experience.Benefits may include:• Medical / dental / vision insurance• Retirement plan with employer match• CME allowance• Paid malpractice coverage• Paid time off
