Compensation & Benefits Salary: $80,000–$90,000 annually, based on experience. Learn more at housingworks.org.About the RoleAs a Registered Nurse, you’ll play a vital role in delivering compassionate, high‑quality care to a diverse patient population. You’ll assess, plan, and provide nursing care in alignment with each patient’s individualized plan, collaborating closely with physicians and interdisciplinary team members. This is an opportunity to practice nursing with purpose supporting clients who rely on our integrated, harm‑reduction–focused model of care.ResponsibilitiesCoordinate comprehensive care plans with interdisciplinary team members.Deliver specialized nursing care and actively build new clinical competencies.Maintain accurate, timely documentation of all nursing activities.Communicate essential updates to the Nurse Care Coordinator and care team.Participate in case conferences to ensure continuity and quality of care.Collaborate with patients and providers to promote optimal health outcomes.Provide reliable on‑call coverage as needed.Build strong relationships with external providers, hospitals, and clinics.Triage patient needs and adjust care plans based on clinical assessments.Address barriers to treatment, including polypharmacy risks and ADL challenges.Administer medications and treatments as prescribed.Conduct nutritional assessments and provide patient education.Follow infection control and safety protocols.Provide emergency care and maintain current CPR certification.Support onboarding and precepting of new nursing staff.Ensure compliance with State, Federal, Medicare, and Medicaid regulations.Perform additional duties as assigned by nursing leadership. QualificationsCurrent Registered Nurse (RN) license in New York State.Bachelor’s degree from an accredited institution.Experience with individuals affected by HIV/AIDS, substance use, or mental health conditions is a plus.Background in interdisciplinary care settings preferred.Bilingual (Spanish/English) candidates strongly encouraged to apply.Equal Opportunity StatementHousing Works is committed to creating a diverse and inclusive workplace.
