With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home. You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.In this role, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate thehome care needs of your patients. You will also review patient insurance information, coordinate activities involved ineach individual care plan and make sure that pertinent findings are shared with family and professional caregivers. And allalong the way, you will experience the respect for your opinions and the high level of autonomy you need to do the jobright.To create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the home health plan of care in collaboration with the physicianTo effectively case manage the care and services for assigned patients resulting in positive clinical outcomes and minimized risks of rehospitalizationSupports agency established financial goalsDemonstrate expert knowledge unique to the practice of community health nursingExcellent clinical assessment skillsStrong ability to solve problems independently and interact with an integrated teamCurrent NYS RN license and registrationBachelor's degree in nursing preferredMinimum of one year acute medical-surgical nursing experienceCHHA experience preferredDriver license and car required
