Active West Virginia state PT license required to start the assignment. BLS certification from AHA or ARC may be required. Experience in skilled nursing facility settings preferred. Ability to provide documentation of professional credentials, immunizations, and background checks as required by facility. Assignment Details Facility Type: Skilled Nursing Facility. Shift Name: Days. Shift Details: Monday–Friday, 0800–1630. Schedule: Five 8-hour shifts per week, 36 guaranteed weekly hours per client notes. Assignment Length: 13-week contract. Split position between local sites; candidate may be required to float between facilities within the region. Meal break: 30 minutes.
