Job DescriptionDaytime Schedule • Preventive Care Focus • Team-Based Practice**Townsen Physician Group | Bellaire, TX 77072Townsen Physician Group is seeking a dedicated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Bellaire, TX practice. This role offers the opportunity to provide comprehensive, patient-centered care across the lifespan in a collaborative, supportive clinical environment.Position SummaryThe Advanced Practice Provider delivers high-quality, evidence-based care focused on preventive health, chronic disease management, and care coordination. Working closely with physicians, specialists, and interdisciplinary team members, this role emphasizes continuity of care, patient education, and improved long-term health outcomes for diverse patient populations.Key ResponsibilitiesPerform and promote preventive screenings (health assessments, physical exams, etc.) in accordance with clinical guidelinesProvide women's wellness services, including:Pap smearsBreast examsContraception counselingMenopause managementDiagnose and manage acute and chronic conditions using evidence-based clinical judgmentDevelop, implement, and adjust individualized treatment plansOrder and interpret diagnostic tests, labs, and imagingPrescribe medications and treatments in compliance with state/federal regulations and company protocolsEducate patients and families on disease prevention, healthy behaviors, and care plan adherenceCoordinate care with providers, specialists, and support services to ensure continuityMaintain accurate and timely documentation in the electronic medical record (EMR)Participate in interdisciplinary meetings and quality improvement initiativesPerform other related duties as neededKnowledge, Skills & AbilitiesStrong clinical assessment, diagnostic, and decision-making skillsSolid understanding of chronic disease management, preventive care, and health maintenanceKnowledge of current evidence-based guidelines, screening recommendations, and immunization schedulesExcellent communication and interpersonal skills with diverse patient populationsAbility to adapt quickly to changing priorities or patient conditionsAbility to work both independently and collaborativelyProficiency with EMR systemsStrong attention to detail and organizational skillsProficiency in Microsoft Office (Outlook, Excel, Word)Compassionate, energetic, and patient-focused approach to careEducation & Experience RequirementsNurse PractitionerActive Texas APRN licenseNational certification as a Family Nurse Practitioner (FNP-BC or FNP-C) through ANCC or AANPPrescriptive authority from the Texas Medical BoardCurrent, unrestricted DEA certificateBLS certificationPALS certificationMinimum 2 years of family or primary care experiencePhysician AssistantActive Texas PA licenseNational certification as a PA-C through NCCPAPrescriptive authority from the Texas Medical BoardCurrent, unrestricted DEA certificateBLS certificationPALS certificationMinimum 2 years of family or primary care experienceAbout Townsen Physician GroupTownsen Physician Group is committed to delivering compassionate, coordinated, and high-quality care to patients and families throughout the communities we serve.
