This full-time, on-site position focuses on adult and elderly patients with chronic and complex medical conditions, including cardiovascular and vascular disease.The ideal candidate is comfortable managing chronic illness, coordinating interdisciplinary care, and promoting preventive health strategies.Practice LocationsDunn, NC 28334Oxford, NCRaleigh, NC 27617Key ResponsibilitiesPerform comprehensive medical evaluations, including histories and physical examinationsDiagnose and treat acute and chronic conditions such as diabetes, hypertension, coronary artery disease, and vascular disordersDevelop and implement individualized care plans for complex patientsProvide preventive services including vaccinations, screenings, and lifestyle counselingCoordinate care with specialists, therapists, and social workersEducate patients and caregivers on disease management and long-term wellnessMaintain accurate and compliant medical documentation (HIPAA standards)Respond to patient concerns and urgent matters, including in-home visits if requiredApply evidence-based practices in cardiology and vascular medicineParticipate in quality improvement initiatives and interdisciplinary team meetingsQualificationsGraduate of an accredited Physician Assistant or Nurse Practitioner programActive North Carolina license requiredPA-C certification preferredMinimum 1 year of cardiology experience preferredNew graduates with a strong interest in geriatrics and cardiovascular care are encouraged to applyExperience managing chronic disease and geriatric populationsProficiency in EMR systems and ICD-10 codingStrong communication skills, empathy, and collaborative mindsetCommitment to professional development and continuing educationCompensation & BenefitsCompetitive annual salary: $125,000 $160,000401(k) with employer matchComprehensive medical, dental, and vision insurancePaid time off and holidaysCME allowanceMalpractice insurance coverageScheduleMonday through FridayOn-call responsibilities requiredThis is an excellent opportunity for a provider seeking to grow within a supportive cardiovascular team while making a meaningful impact on adult and geriatric patient populations.Applicants interested in delivering high-quality, compassionate cardiovascular care are encouraged to apply.
