Clinical Pharmacy Specialist
Job Description: Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‐level assessments aligned with evidence‐based standards.Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.Support quality‐improvement and REMS initiatives.Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.Requirements: Hold a Doctor of Pharmacy (PharmD)Must be a licensed pharmacist in WashingtonCandidate must meet one of the following qualifications:Completion of a PGY2 Ambulatory Care ResidencyCompletion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)At least 3 years of ambulatory practice with disease state managementStrong communication, organization, and tech skillsEmpathy, integrity, and a team-first mindsetBenefits: $10,000 Sign-on BonusSalary + BonusMedical, dental, and vision insurance starting day one401(k) with match, 4 weeks PTO, 10 paid holidaysPaid parental leave, short-term disability, and professional developmentAccess to employee resource groups and wellness programs
