Washington Oncology Clinical Pharmacy Specialist is a remote, full-time position responsible for providing comprehensive medication management and collaborating with care teams to enhance patient care in oncology.
Key ResponsibilitiesProvide comprehensive medication management, including dose optimization and adherence support
Collaborate with physicians and interdisciplinary teams to create individualized care plans
Monitor therapy effectiveness and safety, and support quality improvement initiativesRequired Qualifications, Training, and EducationDoctor of Pharmacy (PharmD) degree
Licensed pharmacist in Washington
Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with 3 years of clinical oncology experience
At least 3 years of experience as a clinical pharmacist in oncology, including 1 year under a Collaborative Practice Agreement
Strong organizational and technical skills
