(sm)The Field Based Community Health Worker is responsible for identifying and initiating referrals for social service programs, including financial, psychosocial, community and state supportive services. As a Field Based Community Health Worker (CHW), you will act with Medicaid members to ensure to provide home and social resources and member resource education. The Field Based Community Health Worker also addresses social determinants of health such as transportation, housing, and food access.Together, we're removing the barriers that keep people from receiving the kind of quality healthcare that makes a difference. The schedule will be determined by the hiring manager upon hire. This position is a field-based position with a home-based office.
