(sm)The Field Based Community Health Worker is responsible for identifying and initiating referrals for social service programs, including financial, psychosocial, community and state supportive services. As a Field Based Community Health Worker (CHW), you will act with Medicaid members to ensure to provide home and social resources and member resource education. The Field Based Community Health Worker also addresses social determinants of health such as transportation, housing, and food access.Together, we're removing the barriers that keep people from receiving the kind of quality healthcare that makes a difference. The schedule will be determined by the hiring manager upon hire. This position is a field-based position with a home-based office. You will work from home when not in the field.Location: Minnesota with local travel up to 25% and mileage is reimbursed at current government rate.If you reside in Eden Prairie, MN, you'll enjoy the flexibility to telecommute* as you take on some tough challenges.Primary ResponsibilitiesCreate a positive experience and relationship with the membersHelp members set goals and develop a care plan to achieve those goalsProactively engage the members to manage their own health and healthcareAs needed, help the member engage with mental health and substance use treatmentUtilize both company and community-based resources to establish a safe and effective case management plan for membersCollaborate with patients, family, and healthcare providersIdentify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive servicesDocument referrals providedAccountable to understand role and how it affects utilization management benchmarks and quality outcomesYou'll need to be flexible, adaptable and, above all, patient in all types of situationsEngage members either face to face or telephonicallyHelp members set person-centered SMART goals and develop a care plan to achieve those goals with regular follow up calls and ongoing documentation of progress towards goals metAdhere to detailed, specific documentation requirements in the member's health recordProactively engage the members to manage their own health and healthcare using Motivational Interviewing SkillsProvide member education on community resources and benefitsUtilize strong skill sets of managing multiple tasks at a time, being self-motivated, driven toward quality results, managing time well, being very detailed oriented and organized, work well in a team and on your own, and the ability to manage multiple deadlinesProvide member education on community resources and benefitsPartner with care team (community, providers, internal staff)Knowledge and continued learning of community cultures and valuesPerforms other duties as assignedWhat Are The Reasons To Consider Working For UnitedHealth Group?
