You will evaluate patients, develop treatment plans, and guide them through recovery while helping them regain independence and prevent hospital readmissions.Home health allows physical therapists to:Work one-on-one with patientsBuild meaningful patient relationshipsCreate flexible schedulesFocus on functional recovery in real-world environmentsResponsibilities • Perform comprehensive patient evaluations in the homeDevelop individualized treatment plans based on physician ordersProvide therapeutic exercises and mobility trainingEducate patients and caregivers on home exercise programsImplement fall prevention and safety strategiesDocument patient progress in EMR systemsCommunicate with physicians, nurses, and interdisciplinary care teamsAdjust treatment plans based on patient progressMinimum Qualifications • Active Physical Therapist (PT) License in the state of practiceGraduate of an accredited Physical Therapy program (DPT or equivalent)CPR / BLS CertificationValid driver's license and reliable transportationPreferred Qualifications • Home Health experienceFamiliarity with EMR documentation systemsStrong communication and patient education skillsAbility to work independently in the fieldWhy Guardian Angel Home Care?
