Job Description Salary:Physical Therapist- Home Care PRN and or Full TimeRequirements and Responsibilities:Evaluates and treats patients recovering from injury, disease, or surgery in a home care environment.Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.Works cooperatively with physicians, case managers and treating therapists.Monitors, documents, and reports patients' progressand communicates needed health information regarding the patient to all appropriate health care team members.Requires state license to practice physical therapy.Reports to manager of clinical services.You will enjoy:Flexible Schedules (you have the ability to move start times for most patients)One on one patient careMileage reimbursement
