This role offers the opportunity to work with individuals across the lifespan, providing high-quality care that improves mobility, reduces pain, and enhances overall function and independence.The ideal candidate is adaptable, compassionate, and committed to delivering individualized care across a variety of settings.Key Responsibilities:Evaluate individuals across all populations and develop individualized physical therapy treatment plansImplement therapy programs to improve function, alleviate pain, and prevent dysfunctionMaintain thorough and accurate documentation, including evaluations, daily notes, progress reports, and discharge summariesEducate patients, families, and caregivers on care strategies and home exercise programsAssess treatment effectiveness and adjust interventions to achieve optimal outcomesEvaluate patient environments (home, daycare, community) and recommend modifications to support functional independenceCollaborate with interdisciplinary team members to address patient needsManage caseloads, coordinate scheduling, and ensure accurate billing practicesSpecialty & Program Areas:Assist with Custom Wheelchairs (PT/OT collaboration)Assist with Custom Durable Medical Equipment (DME) (OT/PT collaboration)Support Feeding Services (High-Need Area) in collaboration with OT/SLPDeliver Community-Based Services (marketed as community services, not home health) across all disciplinesProvide Telehealth Services across all disciplinesQualifications:Doctorate Degree in Physical Therapy from an accredited programCurrent Arizona Physical Therapy license requiredExperience in pediatrics preferred, but not required; new graduates are encouraged to applyAbility to work with both pediatric and adult populationsStrong communication and interpersonal skillsCompassionate, reliable, and adaptable approachWhy Join Us?Flexible PRN scheduling optionsCompetitive compensationContinuing education opportunities401(k) with company matchingComprehensive benefits package (eligibility based on hours)No weekend commitmentsFull-time and part-time growth opportunitiesWe have meaningful work for you - join our team - Apply Today#LI-LE1
