Resource Navigator
Job Overview
The Resource Navigator plays a vital role in connecting individuals to essential services and support systems within the community. This supports pregnant women, infants, young children (ages 0–5), and families by connecting them to community resources, coordinating care, and reducing barriers to services.. The ideal candidate will possess strong interpersonal skills, a compassionate approach, and experience in social work or related fields. This is a non-clinical, community-based role with experience in community outreach. The Resource Navigator is responsible for assessing client needs, developing personalized plans, and providing ongoing support to promote stability and growth. Responsibilities
Conduct comprehensive resource needs assessments to identify clients' needs. Develop individualized resource plans that align with clients’ goals and circumstances
Link families to housing, food, transportation, childcare, healthcare, and benefits.
Assist clients in navigating social services, healthcare systems, housing resources, and employment opportunities
Complete referrals and follow up with families and providers.
Collaborate with community organizations, healthcare providers, and support networks to coordinate services
Maintain detailed documentation of client interactions, progress notes, and service plans
Educate clients about available resources and empower them with information to make informed decisions
Participate in community outreach and family engagement events
Experience
Knowledge of Section 8 housing programs and workforce development initiatives is a plus
Strong communication skills with the ability to build rapport with diverse populations
Ability to work independently as well as part of a multidisciplinary team
Compassionate attitude with a commitment to client-centered care and community outreach
Job Type: Full-time
Pay: $17.50 - $21.00 per hour
Expected hours: 40 per week

This position requires regular travel to homes and community locations within Monroe and surrounding parishes. Are you willing and able to perform field-based work?

This is a non-clinical role and does not provide therapy or counseling. Are you comfortable working in a care coordination and resource navigation capacity?
