Assists the their supervisor in the assessment process for all prospective residents (requesting assisted living services) and current residents receiving assisted living services. Maintains ongoing awareness of data related to residents' health profiles, functional and psychosocial needs. Reports significant changes in resident condition in appropriate time frame to their supervisor and General Manager. Collects and documents information established by regulation and company standard related to resident health, function and psychosocial needs and service preferences. Assists their supervisor, along with General Manager, Sales team, resident, and resident family or other representative that the resident chooses, in designing a Service Plan which mediates department's service provisions related to resident's needs and preferences. Uses company and regulatory standards for maintaining a resident record. Ensures that all staff follows standards in documenting resident activity and service provision. Performs and provides all personal care services required for any resident. Assists resident and/or family identify community resources to support resident health/psychosocial needs.Provides additional management reports as required.If you are someone with:Education:First Aid/CPR certification within 30 days of hire where required by state.Completion of dementia training as required by state regulations within six months of hire date if applicable.High School DiplomaLPN preferredExperience:2+ years experience in Senior Living (Assisted Living, Active Living, and/or Memory Care)Experience in assisted living, geriatric nursing, or gerontology and dementia care.Experience in medication administration and hands-on personal care preferred.Demonstrated ability in budgeting and cost control.Experience in operating an electronic care system.Physical Requirements: This role involves frequent standing, walking, lifting (up to 75 lbs), and assisting with resident care. Regular bending, reaching, and physical movement are required throughout the workday.
