You will develop and maintain rapport with individuals, provide comprehensive case management support, and help connect clients with vital resources and services. May be required to travel and transport patients in your personal vehicle depending on position. Preference will be given to candidates with no RTO and the ability to start on 5/4. Advanced notice will be given for assignment location each day, and the ability to travel to all locations within a 35 mile radius is required.Key ResponsibilitiesDevelop and maintain rapport with individualsProvide telephone outreach and home/community visits if neededProvide hands-on case management support to clients, including providing short-term transportation to support engagement with providers and social servicesLink individuals to resources and valuable information to improve health and wellnessHelp advocate for individuals to improve their access to careComplete required written documentation per program expectationsPerform case management tasks utilizing life experiences to help and aid the understanding of both the consumer population and to staffProvide peer support services such as information, training, and advocacy to assist individuals regain control over their own lives and their recovery processFacilitate mutual support and self-help groupsDemonstrate an ability to develop respectful interpersonal relationships with consumers while maintaining appropriate boundariesHelp guide consumers in developing WRAP and or other wellness plansProvide supervision for consumers while on the unitAssist consumers with researching placement and benefit optionsAssist consumers with applying for and securing housing and benefitsAssist with and contribute to multi-disciplinary planningRespect diverse views and approaches, demonstrate Standards of Respect, and contribute to creating and maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitorsEducation RequirementsAssociate degree in Human Services or HSD or HISETRequired ExperiencePrior experience living with addiction/recovery is requiredPrefer previous experience in human services and an understanding of addiction and recoveryWeekend & Float RequirementsFlexibility is key. Must be willing to work as needed40 hours per week guaranteedMust be willing to float to all CHL locations.
