Coverage Needed: Schedule: 8:00 am - 5:00 pm Call as needed for Cath Lab / Office Exact Dates: October 23-27 November 6-10 November 20-24 November 27-Dec 1 Clinical Information: EMR= EPIC Support Staff: Mix of RNs and MAs 6 Physicians and 3 APPs Will work in Inpatient and Outpatient Special procedures: Interventional Required: Interventional Cardiology skills required &All typical procedures necessary Must be Board Certified Must have IN license to be considered and presented. CV must bein Month / Year format Must have license in hand at time of submission, or proof that the license application has been submitted Any gaps greater than 30 days will require an explanation on the CV DEA must remain in state to maintain privileges.Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active Current Flu Vaccine Must be fully vaccinated for Covid 19 Exemptions only allowed for religious or medical reasons, once approved by the facility.
