Hospital Access Liaison
The Hospital Access Liaison provides rapid, compassionate support to individuals recently discharged from the Emergency Department or inpatient units. This role focuses on linking high-risk clients to community and behavioral health services, supporting continuity of care, and promoting recovery and wellness.Location- Clinton, MOThis is a full-time, benefit eligible position. The schedule is Monday - Friday. This is a great opportunity for someone who thrives in fast-paced, collaborative environments and wants to make a meaningful difference in the lives of individuals transitioning from hospital care back to the community.Key Responsibilities Provide timely, hybrid response to individuals post-hospital discharge, including short-term interventions for up to 30 days. Co-locate at hospitals as needed and work closely with hospital staff to ensure seamless transitions. Schedule and follow up on client appointments, monitoring engagement 7 and 30 days post-discharge. Maintain accurate, professional client records and documentation in compliance with agency and state regulations. Collaborate with internal programs and multi-disciplinary teams to connect individuals with ongoing services and supports. Provide culturally competent, individualized services and identify appropriate community resources when Compass Health services are not available. Attend monthly meetings and weekly clinical supervision; participate in agency-wide initiatives and travel as needed.Requirements, Skills, Knowledge And ExpertiseMaster’s degree in social work, psychology or related field requiredQUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTSAt least one (1) year experience working in behavioral health requiredWorking knowledge of DSM-V requiredA valid MO driver’s license with agency-established minimum automobile coverage requiredQUALIFICATION(S): LICENSURE/CERTIFICATIONPLPC, LPC, LMSW, LCSW or PLMFT, LMFT, licensed in Missouri requiredYou’ll be a great fit for this role if you: Are passionate about supporting individuals during critical transitions in care.
