Registered Dietitian (PACE Program)Bronx, NY (4 days/week) + White Plains, NY (1 day/week)Monday–Friday | 9:00 AM – 5:00 PMContract (6-month LOA coverage with potential extension)Pay Range: $42 - $46/hour (negotiable DOE)About the RoleWe are seeking a compassionate and clinically strong Registered Dietitian to join a PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you'll provide comprehensive nutrition assessments, care planning, and ongoing support to a frail and elderly population. You'll collaborate closely with an interdisciplinary team (IDT) to deliver high-quality, patient-centered care across clinic, community, and home settings.Key ResponsibilitiesConduct initial and ongoing nutrition assessments (in-person and via telehealth) to evaluate participants' nutritional statusDevelop and implement individualized nutrition care plans, including therapeutic diets and clinical recommendationsCollaborate with physicians, nurse practitioners, and the interdisciplinary team to coordinate comprehensive careMonitor participant progress, outcomes, and adjust care plans as neededProvide nutrition counseling and education to participants, caregivers, and familiesMaintain accurate and compliant documentation in accordance with PACE and regulatory standardsTrack and manage participant dietary needs, including allergies, restrictions, and preferencesEvaluate and coordinate needs for home-delivered meals and nutritional supplementsParticipate in IDT meetings, care planning sessions, and quality improvement initiativesSupport center-based programming including nutrition groups, cooking demos, and educational workshopsConduct meal observations and partner with site staff to optimize dining servicesAct as a liaison with community resources and agencies to support participant needsQualificationsEducation & Credentials:Bachelor's degree in Dietetics or related fieldRegistered Dietitian Nutritionist (RDN) through the Commission on Dietetic RegistrationCertified Dietitian Nutritionist (CDN) – New York StateExperience:Minimum 1 year of clinical nutrition experienceExperience working with geriatric or frail populations preferredHome care or community-based experience a plusSkills & AttributesStrong clinical judgment and critical thinking skillsAbility to work independently while collaborating within a teamExcellent communication and patient education skillsAdaptability, flexibility, and problem-solving mindsetProficiency with documentation and basic computer systemsWhy JoinMake a meaningful impact serving a vulnerable, underserved populationCollaborative, mission-driven interdisciplinary team environmentBlend of clinical care, community engagement, and educationOpportunity to expand experience in PACE and geriatric nutrition
