Physical Therapist - Travel Contract

Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.Active Indiana Physical Therapist license required to start the assignment.BLS certification from AHA or ARC may be required.Home health experience preferred. Experience performing skilled evaluations and home exercise program instruction preferred.Ability to document in electronic medical records required. Experience with home health documentation systems preferred.Valid driver’s license and reliable transportation required for travel between patient homes.
