Our mission, vision and values give the right candidate the chance to make a difference in someone's life everyday.The right services, at the right time, in the right setting.We offer great benefits, competitive salaries, career developement and growth opportunities.Home Health REGISTERED NURSE (RN) CASE MANAGERFull Time/Part TimeJob Description:Responsible for care and documentation of care which meets reimbursement guidelines.Coordinate physical care of the patient with family, patient, physician and Hospice team.Communicate with patients/families at all levels.Communicate with all agency staff.Willingness to travel within the southside to see/meet with patients and their family.Minimal call volume.Job duties:• Follows standard Nursing practices and implements agency policy and procedures.• Assesses patient's physical and emotional status, safety at admission and on an ongoing basis.• Develops comprehensive care plan based on each patient's assessment.• Provides direct care and gives instruction to nursing assistants.• Records and reports changes in condition to the physician.• Participates in IDT.• Documents orders in accordance with regulatory and agency requirements.Job Requirements:Current RN licensure in state of practice, successful completion of a nursing education from an approved school of nursing with one year of clinical nursing experience and at least one year of case management experience.
