Doctor of Medicine | Internal Medicine - General/Other Location: Boston, MA Employer: Lancesoft Inc Pay: Competitive weekly pay (inquire for details) Start Date: ASAP About the Position Description: An established PACE program in Cambridge, Massachusetts is seeking Primary Care providers to deliver coordinated care to elderly patients with complex medical needs. The PACE model focuses on comprehensive, team-based care that integrates medical services, social support, and preventive treatment. Providers work alongside interdisciplinary teams to ensure patients receive individualized and holistic care. This opportunity offers a balanced weekday schedule with no call responsibilities and the ability to work with a manageable patient panel of approximately 70 patients. Compensation: Inquire for details Job Responsibilities: • Deliver comprehensive primary care for elderly PACE participants • Manage chronic and acute medical conditions • Conduct clinic visits, home visits, and nursing facility visits as assigned • Participate in interdisciplinary care team meetings • Coordinate care with nurses, therapists, and social workers • Manage patient communication and follow-ups • Document clinical encounters using Epic EMR • Develop patient-centered treatment plans Qualifications: • MD / DO – Family Medicine, Internal Medicine, or Geriatrics (Physicians) • Certified Nurse Practitioner or Physician Assistant • Board Certified (Physicians) • Active Massachusetts Medical License required • Experience working with geriatric or complex care patients preferred Working Hours: Monday – Friday 8:00 AM – 5:00 PM No Call Skills: • Geriatric medicine • Chronic disease management • Interdisciplinary care coordination • Preventive medicine • Epic EMR proficiency • Patient-centered care delivery
