**Job Summary and Responsibilities******This position will service Nebraska City for both Hospice and Home Health.******As a Home Health RN, you will:**+ Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.+ Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.+ Collaborate and coordinate. Work seamlessly with a multidisciplinary team - physicians, therapists, and support staff - to ensure holistic care.+ Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.+ Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.+ Innovate with technology.
