Director of Day Habilitation

Job Description - Receive and review all referrals for day Habilitation services at Folak healthcare, LLC.- Participate in the admission ,suspension and discharge processes for individuals.- Serve as the secondary contact for individuals,families, and Dsp receiving services through day Habilitation.- Review and approve schedules and any schedule changes- Oversee and manage transportation routes for individuals receiving transportation inside and outside of the day program.- Supervise and manage the daily operations of services and support provided by the agency.Review and approve daily services logs for services rendered by employees.- Serve as a member of the agency's Quality management committee to support ongoing growth and compliance,- Assume 24hrs on-call responsibilities as required.- Oversee the administration and safeguarding of medication.- Document , report and follow up on all agency Unusual incident reports.- Cooperate with any internal or external inspections or investigations , including those conducted by federal or Local government investigators.- Respect the confidentiality of all information related to individuals served and their families.- Protect the dignity of the individuals served.- Perform other tasks as assigned. - Company DescriptionFolak is a Day Habilitation program for individuals with developmental and intellectual disabilities.Company DescriptionFolak is a Day Habilitation program for individuals with developmental and intellectual disabilities.
