We are committed to providing compassionate, high-quality care while creating a supportive environment for our staff.We are seeking an experienced Discharge Planning Coordinator to oversee the discharge process for our Sub-Acute Unit and help residents transition safely and successfully to the next level of care.Responsibilities: Coordinate and oversee resident discharge planning for the Sub-Acute Unit Participate in utilization review and insurance/Medicare reviews Issue Medicare cut letters as needed Collaborate with interdisciplinary teams on treatment and discharge plans Provide social service support to residents and families Coordinate community resources and post-discharge services Maintain accurate records and documentationQualifications: Master’s Degree in Social WorkOR Active NJ LPN or RN license Minimum 4 years of social work or clinical nursing experience, including admissions/discharge planning Long-term care or rehab experience preferredWhat We Offer: Flexible scheduling opportunities Outstanding county benefits package Supportive team environment with growth opportunitiesRoosevelt Care Center at Old Bridge is proud to be an Equal Opportunity Employer.
