Join Our Team and Make a Difference in Post-Acute CareWe are seeking a compassionate and experienced Internal Medicine Physician to join our dynamic Post-Acute Division. This full-time position offers a rewarding opportunity to provide high-quality care to our skilled nursing facility patients in the St. Augustine and Jacksonville areas.Internal Medicine Position Responsibilities:Rounding: Monday-Friday rounding on 1-2 buildings per day, depending on census. NP support to assist with patient care. Patient Care: Manage a patient load of 20-22 patients per day. Utilize Nursing Home EMR for efficient charting. Provide comprehensive care to skilled, short-term patients with 3 visits per week. Attend to 3-5 buildings as needed.
