Help us carry out LSS-SW's mission to support and empower our community's most vulnerable members, and our vision of "A future where all people thrive."Position OverviewThe Family Coach delivers mid-level case management and individualized service coordination for families with children ages 0–12. Using a Community Care Model, the role includes assessing family needs, establishing family-centered goals, linking families to supportive resources, and providing ongoing follow-up to promote positive outcomes. The Family Coach also leads parenting education activities, documents progress in the data system, and supports community outreach initiatives. This position is rooted in the Standards of Quality for Family Strengthening and Support and reflects the core values of LSS-SW in all interactions with families, colleagues, volunteers, and community partners.ScheduleThis position is located in East Mesa and requires an eight-hour workday, Monday through Friday, with one to two evenings per week scheduled until 7:00 PM. May work some Saturdays as needed. This position also requires frequent driving to community locations across East Mesa.Key ResponsibilitiesFamily Case Management:Provide mid-level, strength-based case management and service coordination for up to 25 families annually.Conduct family screenings, interviews, and assessments to identify strengths, needs, and priorities.Collaboratively develop individualized family service/action plans based on family-identified goals.Connect families to community resources, including housing, food assistance, parenting programs, healthcare, and mental health services.Provide follow-up support, track referrals, and monitor progress toward goals.Encourage the use of informal supports and promote increased family stability and self-sufficiency.Parent Education and Family Support:Support parenting workshops and family engagement activities that build knowledge of child development and available community resources.Assist with preparing learning environments, session materials, and post-session clean-up.Provide child supervision during adult-only education sessions.Engage and encourage parents during classes to strengthen confidence and peer connection.Reinforce concepts related to child development, family well-being, and positive parenting practices.Promote family engagement through culturally responsive, hands-on learning experiences.Data Documentation & Compliance:Maintain accurate, timely documentation of family services in the Family Resource Center data system.Enter intakes, screenings, action plans, case notes, attendance, referrals, and follow-ups.Ensure data meets program, funder, and audit requirements for quality and accountability.Keep organized, confidential case files.Support data-driven decision-making by reporting accurate service outcomes.Outreach & Community Engagement:Participate in community outreach to promote FRC programs and services.Attend community meetings, events, and resource fairs to represent LSS-SW.Build and maintain relationships with partner agencies and stakeholders.Identify and refer families to the FRC through community interactions.Support collaboration between the FRC and community-based organizations.QualificationsBachelor's Degree in education, social services, behavioral health or a closely related field requiredTwo years relevant work experience is requiredExperience and comfort working with young children birth to 12Knowledge of human service systems and resourcesCurrently have or be able to obtain CPR/First Aid certification and a negative TB testProvide and maintain a current Level One Fingerprint Clearance Card, clear the Department of Economic Security Central Registry Background Check and a criminal background and motor vehicle screeningMust be able to maintain a valid AZ Driver's License, a clear driving record and reliable transportation for the duration of employmentMust be comfortable driving frequently to community locations across East MesaOther Knowledge, Skills, and AbilitiesBilingual English/Spanish is strongly desiredMust be able to travel throughout East MesaThe employee must be able to listen and communicate effectivelyExperience and comfort working with young children birth to age 12 and their parents/ caregiversAbility to work with people from diverse backgrounds non-judgmentally.Ability to effectively present and represent LSS-SW and the program's needs to religious and community groups as well as others in the social services community.Physical DemandsWhile performing the duties of this job, the employee may be required to sit or stand for long periods.The employee frequently is required to use hands to hold objects and operate office equipmentThe employee may be required to occasionally lift and/or move 25 pounds.Reasonable Accommodation Statement: We are committed to providing an inclusive and accessible workplace.
