Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders. Care delivery is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Degree of travel involves home visits and periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients per day. This equates to roughly 6-8 points. Guaranteed hours are not offered. If low patient census affects financial needs, the contractor should discuss floating to a nearby clinic for additional hours. Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. First-timers are not accepted; recent specialty experience is required. Weekend requirements vary by location and profession. On-call requirements vary by location and profession. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Travelers are compensated from patient home to patient home. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted; candidates must have a license in hand. All professionals must be proficient with HomeCare HomeBase (HCHB) and have used it within the last 6 months. If not, they may apply but will require retraining during compliance. RNs must be proficient in OASIS. Travel compliance is required. Must be proficient in OASIS with recent usage or complete a retraining course. All travelers must be proficient in HomeCare HomeBase EMR with recent usage or be required to retrain. No contract hours are guaranteed. Minimum one full year of Adult Home Health or Hospice experience is required. Candidates must have a license in hand, with no exceptions. Candidates cannot currently be an employee of the agency if applying as a traveler. Candidates should confirm their awareness that this is a home health assignment with daily driving to patient homes. Documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care may result in cancellation. Candidates must understand there are no guaranteed hours with this contract. Candidates must achieve weekly productivity points of 30. All overtime must be pre-approved.
