Licensed Practical Nurse (LPN)

Job Purpose: Serves patients by providing educational information; helping with daily living; monitoring vital signs and functions; administering medications. Duties: Administer oral, topical, and injectable medications, including insulin Monitor and record vital signs Perform wound care and dressing changes Provide treatments such as catheter care and tube feedings Observe and document changes in residents’ physical or mental condition Report significant changes to the RN or physician Assist in developing and updating care plans Respond to emergencies and changes in condition Maintain accurate medication administration records (MARs) Chart treatments, assessments, and resident responses Ensure compliance with facility policies and state regulations Supervise and delegate tasks to Certified Nursing Assistants (CNAs) Communicate with interdisciplinary team members Educate residents and families about treatments and medications Support residents’ overall comfort, safety, and quality of life Skills/Qualifications: LPN License in New York State (NYS) CPR/BLS Certification Strong clinical skills, including: Medication administration Wound care Infection control Basic resident assessment Documentation Knowledge of chronic conditions common in older adults Familiarity with nursing home regulations and facility policies Clear communication skills Teamwork and collaboration with interdisciplinary staff Ability to supervise and delegate tasks to CNAs Time management and organizational skills Critical thinking and sound clinical judgment Patience, compassion, and empathy in resident care Ability to provide safe, effective, and person-centered care to elderly residents
