Job Brief1 OpeningPay Range: SOCIAL WORKER - $31.16 to $49.86Job Summary:The EmPATH Social Worker is a member of a multi-disciplinary team responsible for providing social work-related services to the adult population in the crisis stabilization unit. The Social Worker coordinates care of the patient by interfacing regularly with various members of the patient's multi-disciplinary treatment team-Emergency Department providers, psychiatrist, program manager, clinical RNs and staff. The Social Worker provides assessment and case management services, including biopsychosocial and behavioral health assessments, treatment/discharge planning, group, individual and family therapy. They provide assistance to both patients and families to meet related social and emotional needs for psychiatric condition, treatment and recovery.Licensure and Certification:Current BLS provider cardCurrent CA LCSW preferredLanterman-Petris-Short (LPS) Certificate required within 60 days of employment.Education:Master's Degree in Social WorkExperience:One year of hospital and/or mental health social work experience required.Care management experience preferred.Preferred 2-3 years in Behavioral Health related fieldKnowledge and Skills:Strong verbal and written communication skillsStrong organizational skillsProblem solving skillsPatient-focusedAbility to navigate and connect successfully with outside provider agencies (Health Plans, DHHS).Physical Demands - Clerical/Administrative Non-Patient Care:Frequent sitting and standing/walking with frequent position change.Continuous use of bilateral upper extremities in fine motor activities requiring fingering, grasping, and forward reaching between waist and chest level.Occasional/intermittent reaching at or above shoulder level.Occasional/intermittent bending, squatting, kneeling, pushing/pulling, twisting, and climbing.Occasional/intermittent lifting and carrying objects/equipment weighing up to 25 pounds.Continuous use of near vision, hearing and verbal communication skills in handling telephone calls, interacting with customers and co-workers and performing job duties.Key for Physical DemandsContinuous66 to 100% of the timeFrequent33 to 65% of the timeOccasional0 to 32% of the time
