Social Worker - Cardiac Transplant Clinic

Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients' health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity, health system and community.Job Duties:Monday through Friday 8-4:30. Tuesday meetings at 7 am and occasional other AM meetingsInterviews and assess assigned patients to identify areas of needProvides resources and support to patients, families and caregivers as neededDocuments in the electronic medical record appropriately and timelyCommunicates and collaborates regularly with multidisciplinary team;Assists in development of the discharge plan;Makes post-acute referrals and follow through to completion, including placements and transportation;Mitigates discharge delays through timely interventions.Completes family therapy sessions with patients and their families. LMSW or equivalent clinical licensureMaster's degree in social work (MSW)Minimum of 2-3 years of experience in a healthcare setting Job Requirements Applicable Experience:Less than 1 yearLicensed Masters Social Worker (KS) - Kansas Behavioral Science Regulatory Board, Licensed Masters Social Worker (MO) - Missouri Division of Professional RegistrationMaster's Degree Job Details Full TimeDay (United States of America) The best place to get care.
