We specialize in providing support for LGBTQ+ and BIPOC youth, providing safe housing and creative outlets.Position SummaryFull-Time Case Manager with a certified Community Health Worker certification (CCHW) is a key support role focused on improving the well-being of transition-aged youth living in Haus of Codec’s Emergency Shelter and Transitional Housing residence . This position combines traditional case management with a public health lens, helping clients navigate healthcare, housing, and social services while also promoting safe and healthy living conditions.You’ll work closely with clients in our Emergency Shelter program to achieve health and wellness goals, connect them to services, and build community through outreach and engagement. A major part of this role includes conducting direct outreach and being a familiar, trusted presence at community sites and events.Key ResponsibilitiesCase Management & client supportConduct intakes and develop individualized service plansAssist clients with housing navigation, job search, education, and public benefitsSupport access to primary care, behavioral health, and other community servicesDocument progress and maintain case files using internal systems and the Homelessness Management Information System (HMIS) / ClarityParticipate in regular check-ins and care coordination meetings internally and with providers at other organizationsAttend apartment viewings and provide logistical support for clients moving out of Emergency Shelter when neededPromote awareness of how the physical and social environment impacts healthHelp clients set and work toward personal health-related goalsConnect residents to resources through the Healthy Communities Office, Arts & Health initiatives, and Community Health Worker Association of RI (CHWARI)Engage clients with activities that build community and resilienceProvide timely support in crisis situationsAdvocate for clients with service providers, landlords, and public agenciesModel trauma-informed and harm reduction approachesAttend team meetings, supervision sessions, and professional development trainingsHours12-8 PM, Monday-Friday, with some flexibilityQualificationsRequired:CHW Certification (CHWARI CORE) or enrollment in a CHW programFluency in English (verbal and written)Experience in case management, community outreach, healthcare, or social servicesPassion for youth empowerment, especially within LGBTQ+ and BIPOC communitiesStrong interpersonal and communication skillsComfort supporting individuals with mental health or substance use challengesCompetency using Google Workspace and general office toolsAbility to lift up to 50 lbs and work on-site/in the fieldExperience handling confidential informationAbility to work on-site at our residence or downtown office (monthly bus pass will be provided)Preferred:Experience living in or working with people living in public housingBilingual in additional languages (especially Spanish, potentially Haitian Creole, Cape Verdean Creole)Valid driver’s license and access to a carSelf-starters and community-minded folks encouraged to applyKnowledge of harm reduction principles, Housing First, and practices of cultural humility Commitment to EquityHaus of Codec is an equal opportunity employer.
