Case Manager - Adult Program

Crystal Springs is looking for a dedicated and experienced the CaseManager who will is responsible for providing case management servicesto the individuals and their families; initial and on-going collateralwork with licensing agencies, funding sources, referral sources,guardians, courts, and other programs; coordination of internal servicesthat include quality assurance, conducting pre-placement interviews andsupporting the intake process; coordinating and facilitating individualtransitions to new homes within the program and/or to other agencies;and maintaining and/or maintaining needed resources. The Case Managerparticipates collaboratively with all members of the management team.Standards of Care: Promote and support each individual\'s Quality ofLife by assuring Respect and Dignity, Individual Choice, Personal Growthand Accomplishment, Personal Well-being, and CommunityMembership/Relationships Address the total individual, inclusive ofmedical, psychosocial, behavioral, and spiritual needs Involve theindividual, to greatest extent possible, and caregiver in the decisionmaking process Facilitate transitions of care, which includes a completetransfer to the next care setting provider (internal and external) thatis effective, safe, timely, and complete Provide professionalpresentation when representing the agency with external parties Ensurethat all family members, guardians, and visitors are welcomed andtreated with dignity and respect Work collaboratively with fundingagencies, licensing agencies, and guardians in the best interest of theindividual and alert appropriate administrator(s) in the identificationof potential conflicts Ensure visits, telephone, and mail contact withfamily or other identified supportive adults in accordance with theindividual service plan and program rules and standards Promoteinterventions that promote the development of independence and improvedquality of life Maintain professional boundaries and confidentialitywith individuals, families, guardians, and other external stakeholdersEstablish and maintain a positive collaborative working relationshipwith families/guardians, day habilitation program, residential supports,nursing department, providers and other appropriate agencies CaseLiaison: Serve as the primary liaison to families/guardians, agencysupporters and provider organizations Communicate treatment progress,issues, concerns, and general status to families/guardians, and otherrelevant external case workers as needed, and minimally once every twoweeks Maintain routine contact with day habilitation program,residential supports, nursing department, providers and otherappropriate agencies Ensure adequate and on-going communication ofprogress, issues, concerns, and general status to all members of theindividual\'s team Provide initial response to most concerns andcomplaints from parents, guardians, workers, and other externalstakeholders, working closely with the Supervisor II\'s, ResidentialCoordinators, and Director of Residential Services DistributeSatisfaction Surveys to parents/guardians as per Crystal Springsprocedure General Responsibilities: Attend team meetings, ISP meetings,in-service trainings, conferences and seminars as required by Directorof Adult Services Assist in admissions process as assigned Cooperate andparticipate in internal investigations Perform other reasonably relatedduty as assigned Qualifications: Bachelor\'s degree in related field ofstudy Minimum of two years case management experience in Human ServiceField Knowledge of Department of Developmental Disabilities and DayHabilitation regulations and CARF standards preferred
