Social Worker-Hospice

Provides psychosocial assessments, counseling and treatment to patients in their place of residence as prescribed by a physician, to assist the patient/family member(s)/caregiver(s) develop effective coping skills in response to the patients diagnosis, treatment and/or life situation. Performs initial assessment within 5 calendar days of start of care to assess and evaluate psychosocial needs and status of the patient and collect data to contribute to the patient plan of care Actively participates in the developing and implementing of the patients plan of care Ensures that changes in patients environmental and clinical status are communicated to IDT members and Agency office staff verbally and in writing in a timely manner Ensures that physician orders are communicated to IDT team members and Agency staff in a timely, accurate, and thorough manner Assists patient and family with MOST form, Do Not Resuscitate status, Advanced Directives, Funeral Planning and other personal and/or legal directives as needs are determined Maintains current knowledge of community resources. Identifies and accesses appropriate external and internal resources to meet patient and/or family needs and to facilitate optimal patient outcomes while home care services are being provided, and for transfer and discharge processes Masters/Bachelorsdegree from an accredited school of social work Has one (1) year of social work experience in a healthcare setting, hospice preferred Valid state driver's license and reliable automobile. Valid auto liability insurance.
