Job Description Join Our Compassionate CommunityThe Medical Social Worker, under the supervision of the Clinical Manager, plays a vital role in the interdisciplinary care provided to patients receiving home health services. This position involves assessing and creating tailored care plans that address the emotional and social needs of patients.In this role, you will also oversee and guide the Social Worker Assistant when they are part of the patient's care plan. Adhering to our organization's policies, healthcare standards, and relevant laws and regulations is essential.Key ResponsibilitiesEvaluate the patient's emotional and social condition in relation to their illness or injury, including their care needs and responses to treatment.Analyze the interplay between the patient's medical and nursing requirements in their home environment, financial resources, and community support options.Implement necessary actions to access community resources that can help address barriers to the patient's recovery.Engage in communication with patients, their families, and colleagues within the organization.Continuously assess the needs of patients and families, focusing on their understanding of the treatment process and coping with emotional and social challenges.Collaborate as a member of the interdisciplinary care team to coordinate care and serve as a resource for other health professionals in identifying and addressing patient needs.Monitor the performance of the Social Work Assistant (BSW) to ensure compliance with company, governmental, and professional standards in delivering medical social services.Maintain and submit required documentation, including case conferences, patient and physician community contacts, visit reports, and progress notes, while collaborating with other healthcare disciplines to optimize patient care.Required QualificationsA minimum of one year of experience as a social worker in a healthcare environment, including home health or hospice settings.Licensure as a Social Worker in the state of practice, as mandated by state regulations.A valid driver's license, auto insurance, and dependable transportation.Current CPR certification.Experience in discharge planning and accessing community resources (housing, shelter, legal assistance, state/federal financial programs, etc.).Knowledge of medications and their proper administration.Proven ability to prioritize tasks, develop action plans, and perform effectively under pressure.Willingness to work flexible hours and travel locally.Maintain current licensure and fulfill mandatory continuing education requirements.Fluency in reading, writing, and speaking English.Additional InformationTB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will need to undergo TB screening.Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver's license and personal vehicle liability insurance. You must maintain insurance that meets or exceeds your state's minimum requirements.Scheduled Weekly Hours1Compensation RangeThe salary range provided below represents a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting.
