Mental Health Professional (CSW, PLPC, LMSW, LMFT, LMHP

RESPONSIBILITIES: 1. Provides Community Support under supervision of an LMHP and APRN. 2. Assists in developing and implementing plans of services and/or social service treatment. 3. Consults with physician, nurses and other clinic personnel and community agency personnel to develop social diagnosis and plan of treatment or service and to interpret psycho-social factors related to illness, treatment and continuity of care needs. 4. Aids in an interdisciplinary or multidisciplinary team approach to diagnosis, plan, and provide treatment. 5. Assists in arranging for and coordination of delivery of services within the hospital and in the broader community to address continuity of care needs. 6. Documents services provided in accordance with clinical standards. 7. Participates as a member of staff, work unit and/or department committee in programs and resource activities. 8. Submits legible logs within with proper documentation and appropriate signatures. 9. Provides leadership and/or assistance in psychosocial rehabilitation groups. 10. Attends bi- monthly staff meetings. 11. Returns pages promptly. 12. Follows all safety rules. 13. Obtains 16 training hours as required for MHP yearly. 14. Actively participates in all evaluations conducted by CLRHC 15. Actively participates in crisis interventions. Minimum Qualifications: 1. Has a Master of Arts degree in a mental health related field: or 2. Has a Master of Arts degree in a mental health related field: or 3. Has a bachelor’s degree and is a college student pursuing a graduate degree in a mental health related field and has completed at least two courses in that identified field; or 4. Has a high school degree or a GED; and (a) has four years’ experience providing direct services in a mental health, physical health, social services, education or correctional setting, or (b) has two years’ experience as a mental health assistant in a mental health rehabilitation setting.
