A company is looking for a Bilingual Care Advocate to assist seniors, caregivers, and families in navigating the healthcare system. Key Responsibilities Establish trusting relationships with members through active listening and empathy Conduct comprehensive assessments to identify barriers to care Develop and execute individualized care plans integrating medical and social determinants of health Required Qualifications Bilingual fluency in Spanish and English 2+ years of experience in care management, patient advocacy, or clinical social work Deep understanding of Social Determinants of Health (SDOH) and experience supporting diverse communities Highly organized with the ability to manage a caseload and prioritize tasks Comfortable navigating EMRs and care-management software
