Key Responsibilities Engage with members to navigate their cancer journey and identify health and social needs Connect members to healthcare and community resources, including advanced care planning support Handle inbound and outbound calls in a structured, high-volume environment while managing documentation and coordination tasks Required Qualifications At least 2 years of experience in a patient-facing role, such as care coordination or healthcare navigation Bilingual proficiency in Spanish and English Experience in a fast-paced, metrics-driven environment Ability to build rapport quickly with members in a primarily phone-based setting Comfort with technology and remote work environments
