Assist with program development, consultation, resource management, coordination of other services, and agencies.
Coordinate and/or support stakeholder meetings and events held in the community or virtually (frequent travel is required).
Attend staff meetings of partner agencies to explain the changes and updates in the Behavioral Health and I/DD system as well as promote best practices as a Tailored Plan.
Educate other employees and the community at large about services, changes, funding source requirements, eligibility, and service availability.
Work with provider agencies and community stakeholders to encourage the whole person care approach to physical and behavioral health services and person-centered planning process.
Requirements: High School Diploma/GED and six (6) years' experience working with individuals with one of the following: Behavioral Health/Mental Health, Intellectual/Developmental Disabilities or Substance Use; OR
Associate's degree and four (4) years' experience working with individuals with one of the following: Behavioral Health/Mental Health, Intellectual/Developmental Disabilities or Substance Use; OR
Bachelor's degree and a minimum of two (2) years' experience working with individuals with one of the following: Behavioral Health/Mental Health, Intellectual/Developmental Disabilities or Substance Use; OR
Equivalent combination of education/experience.
Must have a valid driver's license.
Must reside within Trillium's Northern Region, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, and Warren.
Must be able to travel within catchment as required.
