Independent Living Case Manager

While this position is home-based, the primary office is in Anderson, IN. In this position, you will work with clients in Madison, Hamilton, Tipton, and Hancock counties. You will collaborate with our internal treatment team and community agencies to directly advocate for at-risk children and families, providing important needs assessments, prevention and intervention services, and progress monitoring. This role requires the ability to adapt to a flexible work week, including evenings and potentially weekends.You Will:Direct Service - Direct contact, indirect contact, and brokered services (50%)Visit and assist each client in achieving their goalsBroker support to clientsProvide transportation to clients associated with specific goalsAttend court with clientsRespond to crisis (depending upon referral type)Develop relationships with agencies in the communityWork with other social welfare agenciesRefer clients to community agenciesReport writing, information entry (50%)Email referring agent and supervisor following each contact or attempted contactReview and upload monthly reports to Kidtraks and NYTDMaintain case and agency paperwork according to standardsPrepare written reports for the Department of Child Services / Probation office within identified time framesYou Have:A bachelor's degree in social work or related field is required.You must be at least 21 years of ageWork in-person, no remote work available for this position.A valid driver's license and insurance.
