Nurse Practitioner

ResponsabilitiesProvide primary health care services to patients of all ages in a CHC model of care including health assessments and diagnosis, health care management and therapeutic interventions, screening, referrals, education, follow-up as well as health promotion and the prevention of illness and injury in accordance with the Standards of Practice for Nurse Practitioners.Collaborate with members of an interdisciplinary team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families.Participate in the development, delivery and evaluation of educational and health promotion programs within the community as needed.QualificationsBachelor of Science in Nursing.Registered Nurse in the Extended Class (as recognized by CNO)Masters in Nursing preferredPreference of experience in a community health or primary care settingCPR and First AidKnowledge and experience in health promotion, education and program evaluationKnowledge of community resources, especially in health and social servicesAbility to work within an interdisciplinary teamComprehension of French and EnglishpreferredProficient with computers, Microsoft Office, and digital communication toolsWork ConditionsSalary range: $112,573.94 – $129,891.03HOOPP pension plan and competitive benefitsCondensed work week of four daysAnnual training budget and optional French coursesA criminal records check will be required prior to hiringApply Now!Use the following form to apply for this position online.
